چکیده

onset of the seizures are quite variable in CD as the duration of gluten exposure is important 1 and a correlation can be found between the duration of the disease preceding the diagnosis (and treatment) and neurological findings as detected even years after the diagnosis. 11,12 The two conditions may coexist for a prolong period of time before the actual clinical manifestations which may be till the late middle age as in our patient. Our case demonstrate that neurological symptom such as epilepsy may be the first manifestation of CD and the diagnosis may be often challenging as the frequency of undiagnosed CD in neurological manifestations of unknown origin may be as high as 16%. 6 A high index of suspicion for CD should be borne in mind in patients with epilepsy with non-specific GI or constitutional symptoms. 13 Although routine screening for CD in all patients with epilepsy is not cost-effective, it is reasonable to perform it in patients with neurological dysfunctions of unknown cause 14 and in patients with intractable epilepsy. 8 Early institution of a gluten-free diet in combination with antiepileptic treatment is beneficial as delay in diagnosis of CD in epilepsy patients or poor dietary compliance may adversely influence the overall outcome and lead to complications. et al. Follow-up of muscular sarcoidosis using fluorodeoxyglucose positron emission tomography Sir, Sarcoidosis is an immune-mediated condition affecting numerous organs, especially the lungs, lymph nodes, skin and eyes. 1 Although asymptomatic muscle involvement is common in sarcoidosis (50–80% of histological cases), symptomatic myo-pathy is rare, being encountered in 0.5–2.3% of the patients. 1 We recently observed a case of symptomatic myositis revealing a recurrence of sarcoidosis with favorable outcome after initiation of infliximab; our case is of particular interest, as it indicates that whole body fluorodeoxyglucose posi-tron emission tomography (FDG-PET) is …

High rate of cardiac sarcoidosis presenting with cutaneous plaque type sarcoidosis in 18F-fluorodeoxyglucose positron emission tomography-computed tomography: a case series

INTRODUCTION Myocardial sarcoidosis is known as a significant complication of sarcoidosis, but Holter electrocardiographic monitoring or echocardiograms might not be sensitive enough to detect cardiac involvement. While gallium scintigraphy has been recommended, 18F-fluorodeoxyglucose positron emission tomography-computed tomography might be more sensitive to detect sarcoidosis. CASE PRESENTA...

Bidirectional ventricular tachycardia in cardiac sarcoidosis☆

A 73-year-old man with history of pulmonary sarcoidosis was found to have runs of non-sustained bidirectional ventricular tachycardia (BVT) with two different QRS morphologies on a Holter monitor. Cardiac magnetic resonance delayed gadolinium imaging revealed a region of patchy mid-myocardial enhancement within the left ventricular basal inferolateral myocardium. An 18-fluorodeoxyglucose positr...

Predictive ability of 18F-fluorodeoxyglucose positron emission tomography/computed tomography for pathological complete response and prognosis after neoadjuvant chemotherapy in triple-negative breast cancer patients

Objective The mortality of patients with locally advanced triple-negative breast cancer (TNBC) is high, and pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) is associated with improved prognosis. This retrospective study was designed and powered to investigate the ability of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) to predict pat...

The role of positron emission tomography-computed tomography/magnetic resonance imaging in the management of sarcoidosis patients.

Sarcoidosis is a multisystem granulomatous disease of unknown etiology. The diagnosis is based on clinical and radiographic findings as well as by histopathological findings. Molecular imaging in recent years has made important progress regarding the study of various inflammatory diseases including sarcoidosis. Positron emission tomography (PET) provides an insight in metabolism of this disease...

Sarcoid reaction associated with Merkel cell carcinoma revealed by fluorodeoxyglucose positron emission tomography: a case report

Introduction: Although the association between cancer and sarcoidosis or sarcoid reaction is known, sarcoid reaction associated with Merkel cell carcinoma is rare. Case presentation: We report the case of a 57-year-old Japanese woman with Merkel cell carcinoma in the inguinal area associated with sarcoid reaction. Fluorodeoxyglucose positron emission tomography demonstrated elevated fluorodeoxy...

Complete diagnostic work-up and prognosis prediction for cardiac sarcoidosis with atrioventricular block: Importance of cardiac magnetic resonance image (MRI) and positron emission tomography (PET)

A 56-year old woman complained of chest discomfort and dyspnea on exertion for one month. She had been diagnosed as mediastinal lymph node sarcoidosis through endobronchial ultrasonography guided transbronchial lymph node aspiration. Electrocardiogram at presentation demonstrated sequential first and second degree 2:1 atrioventricular block. Further imaging study with gadolinium enhanced cardia...
